Thank You for requesting access to the Provider Portal.  Unfortunately, at this time we can not approve your request.  To obtain access, you must meet one of the following requirements:

 

  1. Billing Provider must be In-Network
  2. Out of Network providers must have filed a claim within the last 12 months.
